VACCINE AGAINST MELANOMA (MM)
This Wednesday (19/08/2026) the Merck company released information that could be misinterpreted. It is not a preventive vaccine, it is therapeutic.
It is the result of the completion of the phase 3 clinical trial, which shows that it is safe and works well, and better compared to other previous treatments for treating patients affected by MM (melanoma), including those with metastasis, achieving 49% tumor-free after 5 years.
It consists of a personalized vaccine in which the peptides found in Melanoma cells have been selected (V940/mRNA-4159), acting as antigens associated with pembrolizumab (Keytruda), which is an immunotherapy medication.

Updates and new insights in psoriasis (2019)
Therapeutic advances, both topical and systemic, have been intermittent over the past 50 years. As early as 1929, phototherapy treatments such as the Goeckerman and Ingram regimens were introduced, combining coal tar derivatives with ultraviolet radiation.
In 1950, topical and oral steroids were introduced.
In the 1960s, PUVA therapy (psoralens + UVA) was introduced.
In the 1980s, vitamin D3, retinoids, and cyclosporine were introduced.
In the 1990s, narrowband UVB was introduced, allowing treatment for children and pregnant women.
Since the 2000s, biological therapies acting at the immunological level have opened the door to an increasingly optimistic future. Their use is expected to expand as costs decrease, although they are currently mainly restricted to hospital settings for cases resistant to other treatments.
Oral fumarates (2019)
A fumarate recently introduced in Spain, known as “Skilarence” (dimethyl fumarate), currently represents a highly useful therapeutic option:
No known drug interactions. High patient satisfaction due to the absence of the inconvenience associated with topical treatments or the need to travel for phototherapy. Suitable for patients with hepatic or renal conditions, cardiovascular risk, obesity, or hypertension.
It represents hope for a long-term treatment with a good quality of life.
Its mechanism of action is immunomodulatory and anti-inflammatory, involving the suppression or inhibition of interleukins 8, 20, 12, and 23, all of which play a key role in the pathogenic mechanisms of psoriasis.
The treatment protocol is straightforward and familiar in our experience, as we have been working with fumarates since 2005.
